When you apply for a Blue Badge, you will be asked to provide a number of documents to support your application. The documents you need will depend on the criterion you are applying under. You will save yourself time if you make sure you have the documents available before you start your application.
How to get further medical information or summaries
In order to help us assess your eligibility for a Blue Badge, we may require information regarding your medical history. A copy of your medical summary may be required. To obtain a copy of your medical summary please visit your GP surgery.
